Foundation For The Carolinas is offering vision insurance for you and your family through Cigna.

Effective January 1, 2024:
Cigna Vision will be serviced by the EyeCare network.

Comprehensive Plan

The Benefit (per enrolled individual)

One routine eye exam every 12 months
$130 allowance for eyewear every 12 months (including contact lens exam)
A contact lens fitting, re-fit, or evaluation every 12 months

Co-Payment

Exam: $10, Eyewear: $25

Eyewear Allowance

If you exceed the annual allowance for frames and lenses, you will obtain a 20 – 25% discount on the excess amount. You receive a 20% discount off the cost of additional pairs of prescription glasses and non-prescription sunglasses. There is also an average 15% discount for LASIK or PRK from participating providers.
